Where and to whom should I submit my application files?
Application files (Petition and CD) must be delivered in person to the Giresun University Rectorate Private Secretary unit (to Filiz Mert or Kübra Apaydın) during working hours on weekdays.
What should be the content and file format of the application CD?
The CD must contain the following documents:
1) Application petition wet-signed by the principal investigator,
2) Application form with all fields filled out and wet-signed,
3) All other required documents. You can access the documents via the link.
Important: All these documents must be scanned and converted into a single merged PDF file.
Do I also need to send the application file via e-mail?
Yes. You must send the merged PDF file contained in the CD and, additionally, the Word version of the Application Form to tibbiarastirmalarek@giresun.edu.tr. "Ethics Committee Application File" must be written in the subject line of the e-mail.
Whose signatures must be on the application form?
The application petition must be signed only by the principal investigator (GRU staff). However, the commitment section of the Application Form must be signed with a wet signature by all researchers named in the project.
What should be added to the file for studies using Surveys or Scales?
1) For surveys: A blank copy of the survey form must be added.
2) For scales: The source (article, thesis, etc.) proving the validity and reliability of the scale used must definitely be included in the file.
What is the application condition for a multi-center study?
In order for multi-center studies to be evaluated by our committee, it is mandatory that the research coordinator is a Giresun University employee. Additionally, every page of the "Multi-Center Study Protocol" must be signed by the principal investigator, and the approval sections must be prepared separately for each center.
How should I proceed if the Committee requests a correction (revision)?
Revision applications must be made in accordance with the rules of the initial application.
1) A revision petition and the "Revision Form" attached to it must be prepared.
2) The revised version of the application form must be added to the file.
3) The prepared documents must be delivered to the Rectorate as a CD and also sent to tibbiarastirmalarek@giresun.edu.tr with the subject line "Ethics Committee Revision Application".
